CES Vulnerability Review Request

Use this form to request a formal Vulnerability Review for a specific client.
It is appropriate to use this form when:
1. Access Point or Outreach staff have concerns about the vulnerability of a client or accuracy of an assessment.
2. Program staff at a community agency have concerns about the vulnerability of a client or accuracy of an assessment, with permission from their direct supervisor.
